CURRENT ACTIVITIES IN FANTUS

Endocrinology

      1. Dynamic tests clinic (3 patients /week).
      2. Inpatient 131 I therapy for thyroid cancer (1 patient/week).
      3. Complex patients clinics (25 patients/week).
      4. Outpatient 131 I therapy (5 patients/week).
      5. Regular follow-up clinic (70 patients/week).
      6. Hormone injection clinic (testosterone, Somatostatin LAR etc.) clinic (12 patients/week).
      7. Thyroid biopsy clinic (5 patients/week).
      8. New patients clinic (max 33 patient/week).
      9. Combined endocrine surgery clinic (with endocrine surgeons and ENT) up to 6 patients.
      10. Diabetes Podiatry screening clinic (up to 10 patients).

Diabetes

      1. Triage of new referred patients (6 patients a day).
      2. Triage of 16 new walk-in patients a day.
      3. Attending Clinic of complex diabetes patients (max 22/day).
      4. Individual education/case management:
        a. CDE (RD) nutrition and self-management
        b. CDE (RN, APN) self management.
        c. CDE (Pharm.D) medication management.
        d. RN basic self-management.
        e. Health promoter (promotora) self management.
        f. Social Worker case management.
        g. Clinical psychologist: case management.
      5. Classes
        a. CDE: basic skills (3 components)
        b. RN: Glucose monitoring/insulin infections
        c. Health promoters: life style, self-management
        d. CDE: advanced insulin adjustments
        e. CDE: Reinforcement class (slow learners)
